Section 2 — Emergency Medical Authorization

If I cannot be reached and Crafty Makers reasonably believes emergency care is necessary, I authorize Crafty Makers staff to contact 911 and obtain emergency medical evaluation or treatment for my child. I understand that Crafty Makers staff do not provide medical diagnosis or routine medical treatment. I am responsible for resulting medical expenses to the extent permitted by law. Crafty Makers staff do not routinely administer medication. Any emergency medication or accommodation requiring staff involvement must be discussed and agreed in advance with Crafty Makers and, where applicable, the host school.

Section 3 — Photography & Video Permission

Photography & Video Permission *

YES — I give permission for photographs/video of my child to be used by Crafty Makers LLC on its website, social media, and printed or digital promotional materials.NO — I do not give permission for my child to be photographed or recorded for promotional use.

Declining promotional photography does not affect your child's eligibility to participate. Crafty Makers will never use your child's full name with promotional images.

Section 4 — Assumption of Risk & Liability Release

I ACKNOWLEDGE AND UNDERSTAND that participation in arts and crafts activities involves inherent risks, including cuts from scissors or craft tools; burns from glue guns or heated materials; allergic reactions or sensitivities to craft supplies; slips and falls; and other injuries associated with ordinary use of art and craft materials and tools. Age-appropriate safety measures and supervision will be provided, but Crafty Makers LLC cannot guarantee complete safety. TO THE FULLEST EXTENT PERMITTED BY OHIO LAW, for myself, my spouse, each child named in this form, and our heirs, I knowingly and freely assume the inherent risks of participation, including known and unknown risks. I release, waive, discharge, and hold harmless Crafty Makers LLC, its owners, employees, volunteers, agents, successors, and assigns from claims alleging ordinary negligence in connection with each child's participation. This release does not apply to willful or wanton misconduct or to liability that cannot lawfully be waived.

Section 5 — Crafty Makers Policy Handbook

Please review the Crafty Makers Policy Handbook before continuing. It includes important information about attendance, behavior, safety, illness, authorized pickup, late pickup, cancellations, refunds, photography and communications.

Section 6 — Final Agreement & Signature

I confirm that I am at least 18 years old and am the legal parent/guardian of each child named in this form. I have read and understand this Enrollment & Activity Waiver and sign it voluntarily. I understand that Crafty Makers LLC does not provide accident or health insurance for participants and that I am responsible for the medical expenses and insurance coverage of each child named in this form. If any part of this agreement is found to be invalid or unenforceable, the remaining provisions will continue to apply. I understand that this agreement is governed by the laws of the State of Ohio and that any legal disputes shall be resolved in the courts of Hamilton County, Ohio.
